*Mammograms are available to uninsured women ages 40 and older. Women under 40 require a prescription. Advanced registration is required, and space is limited! To register, please call (800) 564-6868 and mention The Actors' Fund event on 11/4/03.

**Prostate cancer screenings, sponsored by the American-Italian Cancer Foundation, are for men ages 40 and older only.

***Vision screening, provided by the Kress Vision Program of NYU Downtown Hospital, will take place at the Al Hirschfeld Free Health Clinic (475 West 57th Street) on November 11, 2003 and is available to documented entertainment professionals who have little or no vision care medical insurance. Registration for vision screening is required and can be arranged at the health fair or by calling the Hirschfeld Clinic in advance, at (212) 489-1939 for an appointment.

Aside for mammograms and vision screening no registration is required.
