Health Enrollment Cards Discontinued

Revised First Rehearsal Questionnaire Now Available

The current Equity Stage Managers Kits have recently been updated. A revised First Rehearsal Questionnaire has replaced the former Health Enrollment cards. Equity members no longer need to fill out the 3 x 5 Health Enrollment Cards, which have been discontinued by the Equity-League Trust Funds. First Rehearsal Questionnaires need to be filled out only by new members on their first contract or by members whose personal information has been changed since their last contract.

The new First Rehearsal Questionnaire is available online in the Document Library. If you have any questions or need forms, please call your local Business Representative.

Contracts, Membership, the Trust Funds and other departments depend on the timely and accurate filing of your contract. Don't miss out on important Health, Pension or 401(k) Benefits - file your contract as soon as possible.

In the Members Only section of the website, you can manage your contact, financial and security information. This is important because bona fide producers and casting directors frequently call Equity for your contact information. You can also pay your Basic Dues and Initiation Fees online using a major credit card or electronic bank check.
