• address and phone number where we can reach you
• social security number
• current mailing address
• date damage occurred
• address of the affected property
• brief description of the damages
• insurance information
• family gross income

If you are eligible for disaster assistance, you may choose to have the assistance check deposited directly into your bank account. Provide this information ONLY when YOU call the number above.

Government disaster assistance covers basic needs only and will not normally compensate you for your entire loss. If you have insurance, the government may help pay for basic needs not covered under your insurance policy. Some disaster aid does not have to be paid back, while other forms of help may come in the form of loans.

Disaster recovery assistance is available without regard to race, color, sex, religion, national origin, age, disability or economic status. If you or someone you know has been discriminated against, you should call FEMA at 800-621-3362 (TTY 800-462-7585) or contact your State Office of Equal Rights.
