If your Equity card is ever lost or stolen, you may obtain a replacement at any Equity office for a $2.00 reprinting fee.
You may make your request by phone; submit a written request; or visit one of our offices. At our Eastern, Central and Western Regional offices, you can get a new card immediately. At the Orlando office you'll receive a temporary card and a replacement will be mailed to you.
