I got an email or a phone call about an ‘Injury/Illness Report Follow-Up.’ What’s that?

Starting in 2009, Injury/Illness Reports are being collected from all Production, LORT and Off-Broadway contracted productions. These reports are filled out by the show’s stage manager and submitted to Equity. The Workers’ Comp department then contacts each member on the report individually about their injury. If time was lost due to injury, we can get the Supplemental information to the members before more time is lost. Please make sure your phone number and email address are current with the Membership department so we can follow-up with you.
